We provide online telehealth services that allow our Members to share their health history and engage with independent healthcare professionals (“Healthcare Professionals”) to receive medical and healthcare services. The Healthcare Professionals who provide healthcare services through our platform are independent professionals affiliated with various independently owned professional practices.

Please note that Users in different regions may be subject to different data protection laws, regulations, and standards. Therefore, this document contains a section specifically dedicated to consumers under the jurisdiction of the European Union.

  • Eligibility, claims, medical history, and other Employer data: If your eligibility for our Services is through your employer, health plan, or labor union (referred to collectively as your “Employer”), your Employer may provide us with data directly or through a third party to verify your eligibility, facilitate communication, and understand your health. This information may include your name, email address, phone number, service eligibility data, carrier membership data, biometric data, medical history and claims data, and identity verification information (such as your birthday, partial social security number, and/or employee ID number).
  • Registration: To access certain services offered on the Sites or elsewhere, we may ask you to register and provide your email address, password, first and last name, and other contact information and personal details. We collect this information for identification purposes, to communicate with you regarding your account, and to facilitate the functioning of certain services.
  • Forms: To fully utilize our services, you may need to complete forms that contain personal information such as your name, address, telephone number, social security number, account information, employment details, health status, and other personal information relevant to your health insurance coverage, diagnosis, and treatment.
  • Surveys: At various points during your experience as a Member, you may have the option to participate in surveys aimed at assessing your service experience.
  • Medical Records: In order to support medical professionals in providing second opinions on your health, you may be required to provide us with your past and current medical records, symptom descriptions, medical history, and lifestyle information.
  • Correspondence: If you communicate with us via postal mail, email, in-app chat or messaging, phone, or video, we may retain records of that correspondence.
  • Technical information and usage information: Similar to other websites, we collect information about users’ devices (such as browser and device types, device identification numbers, and other online identifiers) and their usage and navigation of our Sites. This information helps us improve the design of our Sites to better suit users’ needs. For example, our Sites track the URLs you visited before and after visiting our Sites, as well as your Internet

If your Plan offers incentives for service utilization, we may share information required to implement those incentives. We may also request courtesy notices from your treating physician(s) and additional medical records for the purpose of reviewing the quality of service, understanding treatment outcomes and costs, and improving treatments and recommendations. We will handle this process with care to minimize personally identifying information. Anonymous information gathered, including from medical records, may be used to generate insights about healthcare processes, specific conditions, and other matters. Anonymous information may be published by us, affiliated physicians, and researchers in journals, websites, and other locations. Your name or any identifying information will not be published without your permission.
2. Third-Party Contractors: We may disclose your information to third-party contractors who perform services on our behalf to provide specific services or complete transactions conducted with us.
3. Communication: We may use the information we have about you to contact you about relevant services through various channels such as telephone, text message, email, or direct mail.

Please note that we do collect information from employers regarding Members of all ages to determine eligibility for Services. If eligible under CCPA or GDPR, a parent/guardian may request that we cease processing that information. However, please contact your Employer if you wish for that information to no longer be provided to health carriers.

By choosing to visit DoktorConnect’s Sites, using our Services, or providing us with information, you agree that any privacy disputes or issues related to the terms outlined in this Privacy Policy will be governed by the laws of the State of Texas. You also agree to comply with any limitations on damages specified in our Terms of Service or any other agreement we have with you.

DoktorConnect’s Sites are owned and operated by DoktorConnect. The terms stated in this Privacy Policy remain effective even if the Member’s coverage is terminated, to the extent that we retain information about a Member.

Geographical Restrictions

DoktorConnect does not represent that all products, services, and materials described on the Sites, or the Services available through the Sites, are suitable or accessible for use outside the United States or in all states and territories within the United States.

Disclosures

All Providers offering licensed Clinical services on the Sites hold professional licenses issued by the relevant licensing boards or agencies in the states where they practice. All physicians and psychologists possess advanced degrees in medicine or psychology and have undergone postgraduate training. If you have a complaint related to the care provided by a Provider, you can report it to the professional licensing board in the state where you received the care.

Any clinical records created as a result of your use of the Clinical Services will be securely maintained by DoktorConnect on behalf of DoktorConnect Professionals for a period not less than the minimum number of years required by state and federal law, typically at least six years.

Credit Transactions

DoktorConnect accepts payment only via major credit cards and does not accept cash, check, money order, or insurance reimbursement. By paying with a credit card, you authorize us to charge the indicated credit card for any fees and amounts specified in your order when they become due and payable. If we are unable to debit the applicable amounts from your credit card due to expiration or other reasons, we will notify you, and you must promptly provide a valid credit card account number. Your request for services from the Sites will only be fulfilled upon receipt and verification of full payment.

Other
