Description of Services:

DoktorConnect's Healthcare Professionals offer healthcare services using interactive audio and video technologies, allowing for remote consultations where you and the Healthcare Professional are not physically present in the same location. This enables the establishment of an ongoing treatment relationship with a Healthcare Professional. However, please note that your initial visit may begin as a consultation to assess the most appropriate treatment setting for your care, and it may not necessarily lead to an ongoing treatment relationship. In case of emergencies or when recommended by a Healthcare Professional, it is important to seek immediate help or follow-up care as needed and continue consulting with your other healthcare providers as advised. The Services we provide offer benefits such as improved healthcare access and convenience. However, similar to any healthcare service, there are potential risks associated with the use of technology.

These risks include, but are not limited to:

  1. In rare instances, the transmitted information may be insufficient for making healthcare decisions.
  2. Disruptions may occur due to electronic equipment failures or unstable internet connections. In such cases, alternative means of communication, such as phone calls, may be used to reach you.
  3. In rare instances, limited access to your complete health records may result in adverse drug interactions or other errors.
  4. While we have implemented robust security protocols to safeguard the confidentiality of your health information, there is a slight possibility of security protocol failure, which could lead to a breach of confidentiality.

By accepting this consent, you acknowledge that you understand and
agree to the following:

  1. We may share information about the healthcare services you receive from us with your health insurance company. This may include details related to genetic testing, substance abuse, mental health, communicable diseases, and other health conditions, as outlined in our Privacy Policy.
    If your health insurance coverage does not fully cover the charges for the Services, you may be responsible for partial or full payment.
    Our Health Professionals may share your health records with other healthcare providers involved in the provision, coordination, or management of your healthcare, in order to meet your healthcare needs. This may include information pertaining to genetic tests, substance abuse, mental health, communicable diseases, and other health conditions, as governed by the terms in the DoktorConnect Privacy Policy.
    We reserve the right to determine that our clinical services may not be suitable for some or all of your treatment needs, and therefore, we may choose not to provide clinical services to you through the Site.
    In the context of psychotherapy, you have the right to receive information from your Healthcare Professional about the therapy methods, techniques used, expected duration (if known), and fee structure. You have the option to seek a second opinion from another therapist or terminate therapy at any time.
    By accepting, you confirm that you have the legal authority to act as the guardian or personal representative for all children registered under your Account.
    No guarantees or assurances can be provided regarding the results of the services.

Group Therapy

If you and a Healthcare Professional decide to participate in group or couples therapy (referred to as “Group Therapy”), you understand that the information discussed during Group Therapy is intended for therapeutic purposes and not for use in any legal proceedings involving Group Therapy participants. You agree not to subpoena the Healthcare Professional to testify for or against other Group Therapy participants or to provide records in court actions against other Group Therapy participants. You acknowledge that any information shared individually with the Healthcare Professional by any Group Therapy participant, whether through phone calls or other means, may, at the therapist’s discretion, be shared with other Group Therapy participants. You agree to share responsibility with the Healthcare Professional for the therapy process, including goal setting and termination.
